MIBG  

MIBG is a radiotracer which uses I123  for the isotope which binds to bone marrow.  It is most useful in identifying abnormality in bone marrow of patients who have neuroblastoma and other small cell tumors, primarily.  A bone scan may be negative in these patients, while the MIBG reveals intense activity, both in the primary tumor, as in the case below, as well as in marrow.  It is a useful agent to identify metastatic disease in this disease and is used for regular follow-ups.

This is a 5 week old boy who presented to his pediatrician with recurrent vomiting...an Upper GI Series was ordered.  However, a mass in the chest was noted at the beginning of the study.  Subsequent CT exam of the chest revealed a soft tissue tumor.

The MIBG study showed intense uptake in the mass, consistent with thoracic Neuroblastoma.
